• Fill out the required contact fields 
• Answer the required questions
• Select the Mayflower passenger from whom you think you are descended in the dropdown menu.
• Fill in each generation from the child of the Mayflower passenger down to yourself.  Provide the first and last name of each person in your proposed lineage.  
• Type the full name of the spouse/partner of the line carrier for each generation in the designated field.  
• Fill out your payment info and then click “Submit”
• Please, do not include dates.
• Please, do not include documentation.
• Please, add our email address MLM@themayflowersociety.org to your contacts or safe senders list to prevent your results from being marked as spam or junk mail.

Please note: In order to process your request, we must have your complete line of direct descent (each generation from the Mayflower passenger down to yourself).
